National Patient Safety Agency (NPSA) - improving the safety and quality of
care through reporting, analysing and learning from adverse incidents and
'near misses' involving NHS patients.

Institute for Clinical Excellence(NICE) NICE is the independent
organization responsible for providing national guidance on the promotion of
good health and the prevention and treatment of ill health. On 1 April 2005
NICE joined with the Health Development Agency to become the new National
Institute for Health and Clinical Excellence (also to be known as NICE).

Enquiry into Maternal and Child Health(CEMACH) CEMACH is a self governing body
managed by its own board with members nominated by six Royal Colleges -

Obstetricians & Gynaecologists (RCOG),
Faculty of Public
Health (FPH),
Royal College of
Anaesthetists (RCA),
Royal College of
Midwives (RCM),
Royal College of
Paediatrics & Child Health (RCPCH),
Royal College of
Pathologists (RCPath).
We have an independent Chair appointed by our board. There will be a
national professional advisory committee for each of our major programmes
(maternal, perinatal and child health). There will also be extensive lay and
voluntary sector involvement on the Board and the national professional
advisory committees.

The
National Confidential Inquiry into Suicide and Homicide by People with Mental
Illness(CISH) The National Confidential Inquiry is a research
project funded largely by the National Institute for Clinical Excellence
(NICE). Other funders are the Scottish Executive and the Health and Social
services in Northern Ireland. We examine all incidences of suicide and
homicide that occur under mental health services in the UK.

Enquiry into Patient Outcome and Death. We aim to review medical
clinical practice and to make recommendations to improve the quality of
the delivery of care. We do this by undertaking confidential surveys
covering many different aspects of medical care and making
recommendations for clinicians and management to implement.
